Amps on last, off first.

As to the rest, in practice it doesn't matter, but the most proper way is going downstream - input devices, console, outboard, DSP, lastly amps. Off is of course the reverse sequence.

I agree with the 'downstream' on, 'upstream' off approach, but would argue that it can matter for other devices, for example where the audio may feed other spaces or systems.
 
Personally I like the sound of a 2000 watt square wave hitting the mains. :twisted:

And just because we don't hear the thumps when things pre amps get turned on wrong doesn't mean we aren't eating away at the operational life of components through regular overload...

Consider also things like headphones left plugged into consoles that can go pop...
 
At my school, our on/off sequence is controlled centrally via a Furman PowerLink which in turn sequences 4 different Furman PS-PRO II's, which has 3 different sets of Delayed Outlets.
Power On: Control Rack 1->Control Rack 2->Lectern Rack->Amp Rack.
Within that, its like this:
CR 1: Wireless Mic Receivers+Mixer->CD Player+Effects Processor->DBX DriveRack 4800
CR 2: Amp for Control Room Monitors
Lectern: Wireless Mic Receivers+Assisted Listening Transmitter->Mixer->CD Player
Amp: Amp For High Frequencies->Amp For Lows+Amp For Cluster Lows->Stage Monitor Amp.

All that controlled via one key switch, very convenient.

Truth be told, I'd prefer that order were changed if it was my system...
Based on what you describe, I'd have lectern before CR2 and swap CD player and assisted listening transmitter within the lectern rack...
