What about a break-out box where you power 2 of the 3 120v. lamps in series on the 240v. twist ?.... Assuming they are the same wattage lamp.

Your looking at someone's "Clinch Plate"... A piece of flat metal used to turn the ends of clinch nails over to lock the plywood scabs on to the 1x3's when building theatrical flats.

Your 7,4,1 problem is a pretty good hint of a internal connection issue or stuck button. Power down, take the back off via the TWO flush mount screws, Note how the two main circuit boards want to mesh together at a 90 deg. angle... make sure the two connectors are engaged together all the way...
