I know this thread is really old, but I'm having a similar problem. I'm running an M-Audio Firewire 410 with QLab and a mixer, but QLab is only showing me outputs 1 and 2 to the device. I've tried everything I can think of to get the others to show up. In the 410 control panel, I can make 3/4...
