So sad. His wife worked on the show and according to CBS "Instagram photos suggest he had kids". While there will be a full investigation and we don't want to engage in lots of speculation here. The early audience interviews and video of the accident suggest that his hand just slipped out of the strap. No obvious signs of equipment failure. Also he appears to only have fallen around 15'-20'... A sobering reminder for all of us that small drops are serious. Again, so sad.
