Any 6x12 (6" diameter, 12" focal length) plano convex lens should work. Both lenses are the same, by the way.
One vendor: Altman Lens, 6x12 Plano Convex, for 360/360Q
It may be more cost-effective to buy a used Altman 360Q-6x12 from eBay, in which case you'd get two lenses.

This is the absolute truth! Any time I need lenses, the first thing I do is check eBay for used fixtures. This way, as derekleffew mentioned, you get two lenses, plus all kinds of parts to keep on hand. Especially useful if you buy the same model instrument that you are working on.
