Automated Fixtures Moving Light Fixture Labels

Trying to find a Template for some moving light labels to go on the bases of fixtures with default settings info as well as space for show setting info. I did a search of the forum but couldn't find anything.

Excel or Word, While I can trouble shoot a dimmer rack and get the moving lights working. Making document templates is something I lack skills in.
