I know there are a lot of forms when it comes to a stage management book. I'm in the process of creating a legit prop list. What are some important columns to put on the form? (Prop;location ect.)

Our stage crew had a list grouped by scene with list of props and which character or stage hand was responsible for bringing it on/off stage.

What is it?
Where does it come from? (Stage Right/Left Prop table, weapon rack # 2, etc.)
Does it need a hand off? Who's responsible for that?
Where does it go? (Set on coffee table DSR, Strike to SR Prop Table)
Everyone who touches it between when it goes on and when it comes off.
What page of the script does it come in on?
