Event Advance Sheet/ Rental Application

Im looking into remaking our rental application and also making a new tech sheet. Does anyone have samples that I could take a look at? I want to make a sheet where the renter puts all their tech needs and was wondering what you guys have done in the past. We have alot of dance shows and plays at our theatre.

Any suggestions or examples would be appreciated!

Our tech "sheet" is pretty long and not many actually read the whole thing, but if you list it, they can't say they weren't told !.

It's on our website as well as info about contacting the General Manager, who handles rentals.

This is what one of the venues I work at uses. The application asks for dates/times/equipment usage.

Cost is stated on our fee schedule. Will post later.
