Based on the images I've seen on the news etc, my strong suspicion is that the "transformer" the news people are talking about was probably a 11kV - 400V substation.
Apparently the guys came out covered head to toe in flames, one died at the scene, another later in hospital and the other two as at last night were still in critical condition with burns to much of their bodies.
Arc flash does sound like a probable cause, perhaps it was also an oil filled transformer and the oil went up too...
